Notice of Privacy Practices

Nomad Health & Wellness keeps a record of the care you receive. We are required by law to protect the privacy of that record, to give you this notice of our privacy practices, and to follow the terms of the notice currently in effect.

• For your treatment. Your clinician and care team review your diagnostic results, medications, mobility screening and history to care for you.
• For payment. To bill and collect for the care you receive, where applicable.
• For our operations. Quality review, training, and running the practice.
• With your written authorization. Anything outside treatment, payment and operations — including releasing records to another provider or a family member — needs your signed authorization, which you may revoke in writing at any time.
• As required by law. Public health reporting, court orders, and similar legal obligations.

We do not sell your health information and we do not use it for marketing without your written authorization.

• See and get a copy of your record, including the reports in this portal.
• Ask us to correct information you believe is wrong or incomplete.
• Ask us to limit what we use or share, and to reach you a certain way or at a certain address.
• Get a list of who your information has been shared with.
• Get a paper copy of this notice.
• File a complaint if you believe your privacy rights were violated.

To exercise any of these rights, contact our Privacy Officer at privacy@nomaddlc.com or 434-473-1277. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

• Your record is encrypted in transit and at rest.
• Each account only reaches the records it is authorized to see; front-desk accounts cannot open clinical records.
• Every view, change, upload and download is written to an audit log.
• Sessions sign out automatically after a period of inactivity, and staff devices can require a quick-unlock PIN.
• Notification emails never contain results or clinical details — only a prompt to sign in.

If your unsecured health information is ever breached, we will notify you as required by law.

We may change this notice and apply the change to information we already hold. The current version is always posted here with its effective date.
